The Inner Spectrum Institute is seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to provide essential mental health services to patients and families in our Hospice Center located in Aurora, Illinois. In this full-time remote position, you will play a pivotal role in supporting patients during some of their most challenging times, ensuring they receive the emotional and psychological care they need.

Key Responsibilities\n

• Conduct thorough mental health assessments to evaluate the emotional and psychological needs of hospice patients, considering their backgrounds, medical histories, and end-of-life concerns.

• Develop and implement individualized treatment plans tailored to each patient’s unique experiences, focusing on emotional support, grief counseling, and coping strategies.

• Provide evidence-based therapeutic interventions, including but not limited to cognitive-behavioral therapy (CBT), supportive counseling, and trauma-informed care.

• Facilitate individual and group therapy sessions aimed at helping patients and their families navigate the complexities of terminal illness and loss.

• Offer crisis intervention and emotional support during urgent situations, employing de-escalation techniques and coordinating with other healthcare professionals when higher levels of care are required.

• Educate patients and families about mental health resources, coping mechanisms, and the grief process, empowering them to make informed choices during challenging times.

• Maintain meticulous documentation of all interactions, treatment plans, and progress notes to ensure compliance with legal and ethical standards.

• Participate actively in multidisciplinary team meetings, fostering collaboration amongst medical staff, nurses, and other social workers to enhance patient care pathways.

• Stay informed about current best practices and regulatory changes affecting mental health care, particularly in hospice and palliative settings.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current and valid LCSW licensure in the state of Illinois.

Compensation and Benefits\n
