Key Responsibilities\n

• Conduct thorough and compassionate assessments that identify clients' social, emotional, and environmental challenges, tailoring interventions to meet their unique needs.

• Design and implement individualized service plans in collaboration with clients, establishing measurable goals and regular progress reviews.

• Provide counseling support to patients and their families as they navigate complex emotions and decisions associated with end-of-life care.

• Facilitate crisis intervention and emotional support during urgent situations, employing risk assessment techniques and safety planning when necessary.

• Collaborate with interdisciplinary teams, including healthcare professionals and community organizations, to ensure a holistic approach to patient care.

• Maintain accurate documentation and compliance with ethical standards and policies governing social work practice.

Patient Population / Client Focus\n

Your primary focus will be on individuals in hospice care, including patients with terminal illnesses and their families. You will address emotional and psychosocial needs during one of life's most challenging phases. Your expertise will enable families to communicate openly, process grief, and find solace and understanding through support services.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois is essential.

• Strong understanding of trauma-informed care principles and sensitivity to the needs of patients and families managing loss.

• Substantial knowledge of community resources and social justice issues impacting vulnerable populations.

Compensation and Benefits\n
