Key Responsibilities\n

• Conduct thorough psychosocial assessments using interviews and reviews of medical history to establish emotional, social, and environmental factors affecting patients' recovery.

• Provide empathetic counseling to patients and their families to address adjustment challenges and emotional wellness to foster a supportive environment.

• Collaborate closely with the rehabilitation team to develop cohesive, individualized treatment plans, integrating the psychosocial needs of each patient.

• Facilitate access to community resources including support groups, vocational rehabilitation services, and housing assistance, assisting patients in overcoming barriers to recovery.

• Offer education and ongoing support to patients and family members regarding post-rehabilitation options and resources available in the community.

• Participate actively in discharge planning, ensuring seamless transitions to home environments or other care settings.

Patient Population / Client Focus\n

You will engage primarily with patients in hospice care, many of whom face complex emotional and social challenges related to their conditions. This includes individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and other neurological disorders. Qualifications\n

Compensation and Benefits\n
