Key Responsibilities\n

• Conduct comprehensive assessments to determine the counseling needs of clients, evaluating their mental health and substance use status.

• Utilize DSM (Diagnostic and Statistical Manual) criteria to assess clients for substance use disorders, ensuring they receive the appropriate level of care based on ASAM (American Society of Addiction Medicine) guidelines.

• Develop and implement individualized treatment plans focusing on the substance abuse counseling needs of clients, including specific interventions that promote long-term recovery.
\n
• Create and execute discharge and continuity of care plans, facilitating successful transitions for clients to appropriate levels of support.
\n
• Maintain meticulous documentation of all counseling services, assessments, referrals, and follow-ups in clients' clinical records to ensure compliance and continuity of care.
\n
• Manage an assigned caseload, providing treatment planning and regular clinical charting updates as necessary to track client progress.
\n
• Coordinate support services for clients and their families, documenting all interventions and services provided in clinical records.
\n
• Engage with external referral sources (e.g., legal, educational institutions, employers) to ensure a holistic approach to client care.

• Facilitate crisis interventions and collaborate with a multidisciplinary team to address immediate needs of clients effectively.

This role specifically targets adult clients in a hospice center setting, providing support for individuals facing the dual challenges of mental health and substance use disorders. Qualifications\n

• A Master’s Degree in Social Work (MSW) and active LCSW licensure are required.

• Familiarity with community resources and support systems within Bolingbrook and the wider Illinois area.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
