This position is primarily remote, allowing for flexible working hours while maintaining a structured approach to patient care. You will be part of a dedicated team comprising various healthcare professionals that works within a Hospice Center setting. Key Responsibilities\n

As a Licensed Clinical Social Worker, your responsibilities will include:

• Assessing the counseling needs of each client and developing tailored treatment plans that address their unique challenges.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria to diagnose substance abuse conditions and determine the appropriate level of care based on ASAM (American Society of Addiction Medicine) guidelines.

• Creating and executing the substance abuse counseling components of client treatment plans, ensuring they align with clinical standards.
\n
• Developing comprehensive discharge and continuum of care plans that facilitate patient transition to necessary follow-up care.
\n
• Documenting all interactions, assessments, and interventions in the clients' clinical records, enhancing accountability and compliance.

• Managing a diverse caseload, including regular treatment planning, case management, and adherence to utilization review standards.

• Coordinating support services for clients and their families, ensuring that all provided services are properly recorded.

• Engaging with external referral sources, such as legal representatives, schools, and employers, when appropriate, to provide holistic support.

• Implementing necessary interventions for crisis scenarios, collaborating with a multi-disciplinary team to ensure client safety and well-being.

Your work will directly impact patients dealing with co-occurring disorders, addressing both mental health and substance use concerns. You will provide individual therapy, family counseling, and group therapy sessions tailored to meet the needs of each demographic. Understanding cultural diversity and recognizing unique backgrounds will be paramount in creating effective treatment plans and fostering a respectful therapeutic relationship.

Qualifications\n

The ideal candidate should possess the following qualifications:

• A valid Illinois LCSW license.

Preferred Experience\n
