Key Responsibilities\n

• Conduct thorough assessments of clients’ mental health needs, developing personalized treatment plans that address their unique circumstances and goals.

• Create and maintain schedules for appointments, home visits, and other professional engagements, ensuring accountability and follow-through on care commitments.

• Ensure that treatment plans are executed effectively, adhering to ethical and legal standards in the counseling profession.

• Participate regularly in interdisciplinary team meetings, contributing insights and updates on client progress.

• Document detailed monthly and quarterly progress notes to track patient development and outcomes.

• Organize and facilitate training sessions for team members on effective implementation of program plans and documentation protocols.

• Provide ongoing follow-up care through interviews and additional home visits, guiding clients towards achieving their mental health objectives.

• Serve as a liaison between clients and the broader mental health community, advocating for clients’ needs and developmental goals.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• Must possess an active and unrestricted Clinical Professional Counselor (LCPC) license in the state of Illinois.
