Key Responsibilities\n

• Conduct comprehensive assessments to identify and address the individual needs of clients, tailoring therapeutic interventions accordingly.

• Maintain clear communication with supervisors, family members, and other relevant parties regarding client progress and treatment plans.

• Prepare and share tentative schedules that outline appointments, home visits, and essential observations to facilitate effective oversight of client care.
\n
• Coordinate and perform in-home visits to gather vital information for case assessments, ensuring all necessary documentation is completed.
\n
• Ensure that individualized treatment plans are executed effectively, accounting for the continuous evaluation of their effectiveness.
\n
• Participate actively in Interdisciplinary Team Meetings and contribute to discussions about treatment strategies and client care management.
\n
• Document monthly and quarterly progress notes that reflect clients’ advancements, challenges, and adjustments to their care plans.
\n
• Facilitate training sessions to ensure that staff and team members are competent in the implementation of treatment protocols and documentation practices.
\n
• Provide ongoing support through follow-up interviews and home visits to meet evolving client needs.
\n
• Maintain meticulous records in an electronic format to ensure that information is accessible and comprehensible for all stakeholders involved in the care continuum.

• Act as a liaison with the public and mental health community to support program development and promote organizational goals.
\n
• Assist in creating and improving training materials to enhance service delivery for clients.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population, serving both adults and youths who present a range of mental health challenges, including but not limited to anxiety, depression, trauma, and substance abuse. You will engage deeply with clients in a range of therapeutic modalities designed to empower individuals and foster resilience.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
\n
• A minimum of two years of experience in a counseling context, preferably within a clinical or therapeutic setting.
\n
• Specialized training in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly encouraged.

• A solid understanding of legal and ethical standards relevant to the practice of counseling.
\n
• Exceptional interpersonal and communication skills, with a proven ability to establish rapport with a varied clientele.

• Demonstrated capability to work collaboratively within a healthcare team.
\n
• Proficiency in Electronic Medical Record (EMR) software and familiarity with other relevant technologies.

• The annual salary ranges from USD 90,000 to 110,000, commensurate with experience and availability of caseloads.
