Work Environment\n

This position primarily operates in a remote setting, offering flexibility and the convenience of working from home. Nevertheless, occasional visits to clients' residences may be necessary for assessments and monitoring. Our standard working hours are Monday through Friday, 8:00 AM to 4:30 PM, with some flexibility required to accommodate client needs, and occasional on-call duties may arise for urgent situations. You'll have the opportunity to collaborate with a diverse team of healthcare professionals, including nurses and caregivers, focusing on coordinated care for seniors.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' emotional, mental, and social needs through structured visits and interviews.

• Develop and implement personalized care plans in collaboration with clients and their families, addressing specific goals and preferences.

• Provide ongoing counseling and emotional support to seniors, assisting them through life transitions, bereavement, and health-related challenges.

• Act as an advocate for clients, ensuring their rights are upheld and that they have access to community resources and support systems.

• Engage with interdisciplinary teams to facilitate effective communication and coordinated care efforts, enhancing the overall service delivered to clients.

• Regularly monitor clients’ well-being by conducting home visits, evaluating living conditions, and identifying any potential safety concerns.

• Foster communication among family members involved in care, offering mediation and support during times of conflict.

• Maintain meticulous documentation of client assessments, care plans, progress notes, and interactions in compliance with organizational policies and ethical standards.

Compensation and Benefits\n
