Key Responsibilities\n

• Conduct comprehensive assessments to determine clients' needs, strengths, and challenges.

Patient Population / Client Focus\n

The primary focus of this role revolves around supporting patients with life-limiting illnesses and their families. This includes addressing emotional distress stemming from diagnoses, providing crisis intervention, and facilitating bereavement support. Our patient population is diverse and may include individuals experiencing trauma, anxiety, and depression related to their health circumstances. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• An active and unrestricted Licensed Clinical Social Worker (LCSW) license in Illinois.
