This position is primarily focused on delivering essential mental health support to clients in a hospice care setting, offering both remote and direct assistance. Work Environment\n

This position offers the flexibility of a remote work setting while primarily serving clients associated with a hospice center. Our team adopts a hybrid model that allows you to conduct assessments and consultations via phone or video conferencing, as well as in-person meetings when necessary. You will be part of a multidisciplinary workforce, composed of social workers, psychologists, and healthcare providers, all striving toward comprehensive care delivery. With a structured schedule from 8 AM to 5 PM, Monday to Friday, you will have the opportunity to create a balanced work-life experience while making a significant impact on clients' lives.

Key Responsibilities\n

• Conduct thorough assessments of clients' current psychological and emotional state, determining their needs and crafting personalized care plans.

• Develop and implement support strategies to help clients address and overcome their challenges associated with neglect, abuse, and emotional distress.

• Utilize crisis intervention techniques effectively, aiming to stabilize clients during critical moments.
\n
• Regularly track and evaluate the progress of clients, adjusting treatment plans as necessary to ensure optimal recovery pathways.

• Maintain detailed and accurate documentation of client interactions, including progress notes and treatment plans, while ensuring confidentiality and compliance with HIPAA regulations.

• Participate in case conferences with interdisciplinary teams to share insights and provide collaborative care strategies.

• Refer clients to additional community resources and services that facilitate recovery and enhance quality of life.

• Offer expert testimony when required in legal settings, ensuring that the best interests of clients are represented.

Patient Population / Client Focus\n

Our hospice care focus means that you will primarily serve clients experiencing serious illness or terminal conditions. Your role will entail providing psychotherapeutic support to patients facing not only their health challenges but also the emotional and existential issues that accompany them. Helping clients navigate feelings of anxiety, fear, and grief is vital, as is providing support to their families during bereavement and in care decision-making processes.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Demonstrated clinical experience in social work, particularly in mental health services in hospice or end-of-life care.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 75,000 to 90,000 per year, commensurate with experience.
