Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate patients' mental health status, strengths, and social needs as they approach end-of-life care.

• Provide individual and family therapy, utilizing evidence-based practices such as cognitive-behavioral therapy and solution-focused therapy tailored to the unique challenges faced by hospice patients.

• Develop and implement individualized treatment plans that align with the emotional and psychological needs of patients and their families, incorporating multidisciplinary collaboration with medical and palliative care teams.

• Facilitate group therapy sessions and support groups, fostering a community of understanding and healing among patients and families experiencing loss.

• Offer crisis intervention and risk assessment as needed, particularly for families facing complex grief, and develop safety plans to effectively navigate these sensitive emotional landscapes.

• Maintain thorough documentation in compliance with ethical and legal standards, ensuring that all patient interactions and treatment plans are accurately recorded.

• Engage in continuous collaboration with a diverse treatment team, including physicians, nurses, and other social workers, to ensure all aspects of patient care are addressed holistically.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from $80,000 to $100,000 per year, complemented by a potential sign-on bonus.
