Key Responsibilities\n

• Conduct comprehensive psychosocial assessments and evaluations to understand each patient’s unique situation, mental health history, and social factors impacting their well-being.

• Develop, implement, and review individualized treatment plans in collaboration with the healthcare team, employing evidence-based practices tailored to each patient's needs.

• Provide therapy for individuals and families, utilizing therapeutic techniques that promote emotional healing and resilience.

• Facilitate referrals to community resources, connecting patients with support services such as housing assistance, employment programs, and educational resources.

• Maintain accurate documentation of patient interactions, progress reports, and treatment plans in compliance with HIPAA regulations, ensuring confidentiality and continuity of care.

Qualifications\n
