Key Responsibilities\n

• Conduct thorough clinical assessments to understand clients' needs and mental health status, allowing for accurate treatment planning.

• Actively participate in weekly interdisciplinary team meetings, sharing insights and collaborating with healthcare providers to ensure holistic care for each client.

• Maintain consistent follow-up to track client progress, adjusting plans as necessary to meet changing needs effectively.

• Facilitate communication among clients, families, and other stakeholders, such as educators, to ensure a well-rounded support system.

Patient Population / Client Focus\n

This role will cater primarily to clients facing diverse mental health challenges, including mood disorders, anxiety, trauma, and family-related issues. You will work closely with various demographic groups, including children, adolescents, and families within the community. Understanding cultural sensitivities and adapting strategies accordingly will be key to successfully engaging with clients and achieving desired outcomes.
