Work Environment\n

This position will be conducted remotely, allowing flexibility in work hours. However, it is important that you are available from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed based on client circumstances. Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, integrating emotional, mental, and social factors to create comprehensive care plans tailored to each client.

• Provide ongoing counseling and emotional support to seniors, assisting them in navigating significant life transitions and addressing challenges such as grief, loss, or isolation.

• Advocate for clients by identifying their rights and ensuring they have access to essential community resources, promoting their well-being and quality of life.

• Engage with interdisciplinary teams—including nurses, caregivers, and other healthcare professionals—to coordinate comprehensive care approaches that address our clients' diverse needs.

• Regularly visit clients to monitor their mental and physical well-being, assess living conditions, and address any safety concerns promptly.

• Facilitate family communication, helping to resolve conflicts and ensuring that caregivers are informed and supported in their roles.

• Maintain detailed and accurate documentation, including client assessments, care plans, and progress notes, in compliance with regulatory standards and ethical guidelines.

Qualifications\n
