Bright Star Counseling is seeking a Licensed Master Social Worker (LMSW) to join our dedicated team. This full-time position will primarily focus on providing essential support and guidance to seniors and their families from a remote school-based setting in Arlington, Texas. Work Environment\n

This role offers a remote work environment with a full-time schedule from Monday to Friday, 8:00 AM to 4:30 PM. Key Responsibilities\n

• Conduct thorough assessments to evaluate clients' emotional, mental, and social health, identifying targeted support needs and potential services.

• Develop and implement personalized care plans in collaboration with seniors and their families, addressing their specific goals, preferences, and overall well-being.

• Provide compassionate counseling and emotional support, helping clients navigate the complexities of life transitions, such as health crises, loss, or changes in living conditions.

• Advocate for clients’ rights and facilitate access to community resources, ensuring they receive all the support and assistance available to them.

• Collaborate closely with interdisciplinary teams, which may include nurses, caregivers, and healthcare professionals, to coordinate comprehensive care and enhance service delivery.

• Perform regular wellness visits and virtual check-ins, monitoring clients' living conditions, and addressing concerns to ensure safety and quality of life.

• Facilitate effective communication among family members involved in caregiving, resolving conflicts and promoting a unified approach to client care.

• Educate clients and families about available community resources, support groups, and programs that foster social engagement and improve quality of life.

• Maintain meticulous and up-to-date documentation in compliance with electronic health records (EHR) systems, including assessments, care plans, and progress reports.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
