Key Responsibilities\n

Your primary responsibilities will include:

• Conducting compassionate and ethical individual counseling sessions remotely to assess and address the mental health needs of hospice patients.

• Developing tailored treatment plans based on comprehensive assessments, ensuring they align with best practices and recovery-oriented frameworks.

• Regularly monitoring and updating treatment plans, while documenting progress accurately in our streamlined electronic health record systems.

• Collaborating with interdisciplinary teams, including physicians and nurses, to coordinate comprehensive care for each patient.

You will primarily work with stable, low-acuity adults receiving hospice care. This population may possess various psychological and emotional challenges as they navigate their illness journey. Qualifications\n

To succeed in this role, you must possess the following qualifications:

• A Bachelor's degree in Social Work or a related field is required.

Compensation and Benefits\n

We offer a competitive salary range of $90,000 to $104,000 per year, commensurate with experience and hours worked. Additional benefits include:
