As a member of our remote team, you will operate within a thoughtful and dynamic work setting designed to facilitate effective communication and collaboration with team members. Our flexible schedule, primarily from Monday to Friday between 8:00 AM and 4:30 PM, promotes a work-life balance while still addressing the needs of our clients with occasional evening and weekend hours when necessary. We utilize advanced electronic health record (EHR) systems to streamline client documentation and care coordination.

Key Responsibilities\n

• Conduct in-depth psychosocial assessments to determine the individual needs of senior clients, considering emotional, mental, and social dynamics.

• Develop personalized care plans in collaboration with clients and their families, ensuring that goals align with their preferences and needs.

• Provide supportive counseling to seniors, guiding them through life transitions, health challenges, and family dynamics.

Compensation and Benefits\n
