Mindful Steps Counseling Services is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team in Trenton, New Jersey. Work Environment\n

This role is designed for a remote work setting, allowing flexibility while focusing on the needs of our clients. You will engage with clients through virtual platforms, as well as occasional in-person visits when necessary. The standard work hours are Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of occasional evenings or weekends based on client needs. On-call responsibilities may also be required to address urgent situations promptly.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of seniors, taking into account their individual emotional, mental, and social factors that impact their well-being.

• Develop personalized care plans in collaboration with seniors and their families, ensuring that their specific goals, preferences, and needs are met.
\n
• Provide counseling and emotional support to seniors as they navigate various life transitions, including health challenges and changes in living status.
\n
• Advocate for seniors' rights, facilitating access to necessary community resources and services that support their quality of life.
\n
• Collaborate with interdisciplinary teams, including healthcare providers, caregivers, and community organizations, to enhance the care and support provided to clients.
\n
• Regularly monitor and evaluate clients’ well-being through virtual visits and in-person assessments, addressing any concerns regarding their living conditions and safety.

• Educate clients and families on available community resources, which includes information on support groups, programs, and activities designed to enrich their social engagement.

• Maintain detailed and accurate documentation of client assessments, care plans, and progress notes in accordance with organizational and ethical standards.

Patient Population / Client Focus\n

This position focuses primarily on senior clients, a population that often faces multifaceted challenges such as health issues, social isolation, and family dynamics. Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) in New Jersey.

• Minimum of two years of experience in a social work role, preferably in settings focused on geriatric care or community services.
\n
• In-depth knowledge of social work principles, and familiarity with aging-related issues and resources available to seniors.
\n
• Demonstrated skills in assessment, crisis intervention, and counseling.
\n
• Strong interpersonal and communication skills, enabling collaborative teamwork across various disciplines.
\n
• Proficient in utilizing electronic health record (EHR) systems and other relevant software for client documentation and case management.
\n

Preferred Experience\n

• Experience working in community centers, geriatric care, or other settings providing services to seniors is highly desirable.
\n
• Knowledge of local community services and networking with other providers to facilitate client resources.
\n
• Background in conflict resolution and mediation among families in care settings.
\n

Compensation and Benefits\n
