Key Responsibilities\n

• Conduct thorough psychosocial assessments for senior clients, identifying emotional, mental, and social challenges impacting their lives.

• Provide counseling services to guide seniors through life transitions, including grief, illness, and significant lifestyle changes.

• Act as an advocate for clients, helping them access resources and services in the community, such as transportation, healthcare, and nutrition assistance.

• Coordinate with interdisciplinary teams, including healthcare professionals, to ensure comprehensive care delivery and follow-up.

• Conduct regular check-ins to monitor the well-being of clients, assess their living environments, and address any safety concerns.

• Educate clients and families about community resources that promote engagement and social interaction.

• Create and maintain accurate documentation of client interactions, care plans, and progress notes, utilizing electronic health record (EHR) systems for organization and compliance.

Qualifications\n
