Work Environment\n

This role offers a flexible remote work setting, allowing you to connect with clients at various locations, primarily schools and community centers dedicated to supporting senior citizens. A standard work schedule will be Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed to accommodate client requirements. Flexibility is paramount in this role to ensure continuous and responsive care for our clients.

Key Responsibilities\n

• Conduct thorough assessments of clients' psychosocial needs through one-on-one interactions, ensuring a comprehensive understanding of their emotional, mental, and social situations.

• Develop personalized care plans in collaboration with clients and their families that address specific goals, preferences, and any unique challenges they face.

• Provide counseling and emotional support tailored to seniors navigating various life transitions, such as health changes or loss of loved ones, and facilitate psychosocial interventions to enhance coping strategies.

• Act as a strong advocate for clients, ensuring access to both community resources and legal rights, guiding them in obtaining necessary services such as healthcare, social services, and housing assistance.

• Collaborate with interdisciplinary teams of healthcare professionals, including nurses and caregivers, to coordinate comprehensive care and address any evolving health concerns.

• Perform regular visits to monitor client welfare and living conditions, adjusting care plans as necessary based on observed needs and safety assessments.

• Facilitate effective communication and conflict resolution among family members involved in the care process, ensuring a cohesive support system for the senior clients.

• Educate clients and their families on available community resources, enhancing their social engagement and integration into supportive networks.

• Maintain detailed documentation of assessments, care plans, progress notes, and other relevant interactions, ensuring compliance with ethical and regulatory standards.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A current and valid state licensure as a Licensed Master Social Worker (LMSW).

• Minimum of two years of relevant experience in a social work position, especially within geriatric or community settings.

Compensation and Benefits\n
