Work Environment\n

This position is fully remote, allowing for flexible scheduling while ensuring that our clients receive the continuity of care necessary for their well-being. Our facility operates within a hospice center that values interdisciplinary teamwork, comprising healthcare professionals, nurses, and caregivers who work hand-in-hand to support our clients. While the standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, there may be occasional evening and weekend commitments based on client needs. Additionally, on-call availability may be required to address urgent situations.

Key Responsibilities\n

• Conduct in-depth assessments of clients’ psychosocial needs by considering emotional, mental, and social facets that impact their overall wellness.

• Develop individualized care plans in collaboration with clients and their families, tailored to meet specific goals and preferences.

• Provide counseling and emotional support to clients and their families during life transitions, helping them navigate challenges related to aging.
\n
• Advocate for clients' rights and facilitate their access to vital community resources and services.
\n
• Work closely with interdisciplinary teams to coordinate individualized care and ensure holistic support for our clients.
\n
• Conduct regular home visits to monitor clients’ well-being, evaluate living conditions, and address any safety concerns.
\n
• Mediate and resolve conflicts among family members regarding care decisions and preferences.
\n
• Educate clients and their families about available community resources, support groups, and programs that enhance social engagement.
\n
• Maintain accurate and up-to-date documentation in electronic health record (EHR) systems regarding client interactions, assessments, and progress notes.
\n
• Stay current on social work practices, policies, and regulations to uphold ethical standards and ensure compliance.

Patient Population / Client Focus\n

This position primarily focuses on the senior population within a hospice care setting. As a Licensed Master Social Worker, you will be directly involved with clients facing complex health conditions, life-limiting illnesses, and the end-of-life process. Your role will be to provide emotional support, facilitate necessary services, and advocate for client and family interests, all while fostering an environment of dignity and respect.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Social Worker (LMSW).

• A minimum of two years of professional experience in a social work role, with a preference for experience in a hospice, geriatric, or community care setting.

Compensation and Benefits\n
