This position offers a flexible remote work environment that allows you to connect with patients from the comfort of your own home, while also visiting our hospice center in Silverthorne, Colorado as needed. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients to evaluate emotional, social, and environmental factors impacting their health status and recovery trajectory, utilizing interviews, direct observations, and the review of medical records.

• Provide individualized counseling and therapeutic support to patients and families, focusing on adjustment issues related to chronic illness and end-of-life care.

• Collaborate with the interdisciplinary rehabilitation team to develop tailored treatment plans that consider each patient's unique psychosocial challenges and health goals.

• Assist patients in accessing necessary community resources, including support groups, vocational rehabilitation services, and housing assistance.

• Guide and educate families on community resources and post-rehabilitation services, ensuring they are equipped to provide ongoing support upon discharge.

• Participate actively in discharge planning processes and facilitate smooth transitions to home or alternative care settings.

Patient Population / Client Focus\n

This role focuses on a diverse patient population facing life-limiting conditions, including those receiving hospice care. You will work closely with patients in various stages of their illness, supporting them and their families as they navigate emotional responses to grief, loss, and uncertainty. Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Colorado.

Preferred Experience\n

While at least 1 year of experience in a clinical social work setting is required, candidates with experience in hospice or palliative care settings, as well as knowledge of community resources specific to end-of-life care, will be particularly desirable. Familiarity with interdisciplinary teamwork and electronic health record systems is advantageous.
