Work Environment\n

This position is remote, allowing for flexibility while serving a school-based setting. You will engage directly with seniors and their families through virtual platforms, enabling you to offer support and resources in a convenient manner. The role may also require occasional visits to partnered organizations or community centers within Edison for assessments and follow-ups, bridging the gap between virtual support and in-person care.

Key Responsibilities\n

Your day-to-day responsibilities will include but are not limited to:

• Conducting comprehensive assessments of client psychosocial needs, taking into account emotional, mental, and social factors impacting their lives.

• Collaborating with clients and their families to develop individualized care plans aimed at achieving specific supportive goals, while ensuring the plans are culturally sensitive and aligned with clients’ values.

• Providing counseling and emotional support during significant life transitions, such as loss, health changes, or relocation, helping clients navigate challenges with dignity and respect.

• Advocating tirelessly for clients' rights to access community resources, ensuring they receive the necessary services and support they deserve.

• Working within interdisciplinary teams, collaborating with healthcare professionals, nurses, and caregivers to create a seamless and holistic care experience for clients.

• Regularly monitoring client well-being through virtual check-ins, assessing any emerging needs or concerns and adjusting care plans as necessary.
\n
• Facilitating effective communication among family members involved in the care process, helping to resolve conflicts through empathic dialogue.
\n
• Educating clients and their families on community resources, support groups, and wellness programs that foster social engagement and promote a higher quality of life.

• Maintaining meticulous documentation of client interactions, assessments, and care plans, ensuring compliance with state regulations and ethical standards.

• Utilizing electronic health record (EHR) systems to track client progress and maintain confidentiality in accordance with HIPAA regulations.

Patient Population / Client Focus\n

The ideal candidate will focus primarily on seniors, understanding the unique and often multifaceted challenges they face. You will work closely with elderly clients experiencing issues related to aging, such as mental health concerns, social isolation, chronic health problems, and transitions in living situations. Your empathetic approach will be key in building trust and ensuring that their needs are met holistically.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is mandatory.

• A current and valid LMSW license in the state of New Jersey is required.

Compensation and Benefits\n
