Key Responsibilities\n

• Conduct detailed assessments to evaluate the psychosocial needs of seniors, identifying factors that influence their emotional and mental well-being.

• Develop and implement tailored care plans in collaboration with clients and their families, ensuring strategies align with their personal goals and preferences.

• Provide individual and family counseling, offering emotional support during significant life transitions, such as health crises or loss of autonomy.

• Act as an advocate for clients, helping them navigate access to community resources and comprehensive services, thereby promoting their rights and independence.

• Monitor client progress through regular visits, addressing any changes in their living conditions or psychosocial status while ensuring safety and satisfaction.

• Facilitate productive communication among family members involved in care, helping resolve potential conflicts and misunderstandings.

• Educate clients and families about available community resources, support networks, and social engagement programs to promote overall quality of life.

• Maintain thorough documentation in electronic health record (EHR) systems, tracking assessments, care plans, and client progress notes to ensure compliance and optimal care delivery.

Patient Population / Client Focus\n

This position primarily focuses on working with older adults who face challenges related to aging, including mental health concerns, chronic illnesses, or social isolation. You will work closely with seniors and their families, providing empathetic support and practical resources to enhance their well-being and quality of life. By understanding the unique perspectives and histories of your clients, you will be better equipped to address their specific needs effectively.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in New Jersey.

• A minimum of two years of professional experience in a social work capacity, particularly within community or geriatric settings.

• Exceptional assessment and counseling skills, combined with the ability to create rapport with clients.
