Work Environment\n

This role is primarily remote, allowing for flexibility in how and where you accomplish your work. While most interactions will be conducted via telehealth platforms, occasional travel may be necessary to perform in-person assessments and support for clients, addressing their immediate needs. Working hours are primarily Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends required to accommodate client schedules. The role demands a degree of adaptability to respond to the varying needs of clients.

Key Responsibilities\n

The Senior LMSW will be responsible for a comprehensive range of tasks, including but not limited to the following:

• Conducting thorough psychosocial assessments to identify clients' emotional, mental, and social needs.

• Developing and implementing personalized care plans in collaboration with clients and their families, addressing specific needs and goals.

• Offering ongoing counseling and emotional support to help seniors navigate life transitions, including health changes and family dynamics.

• Advocating for clients to ensure their access to necessary community resources and services, addressing barriers to care.
\n
• Actively collaborating with an interdisciplinary team, including nurses, caregivers, and other health professionals to enhance the comprehensive care provided to clients.

• Monitoring clients’ well-being through regular check-ins, home visits, and prompt response to emerging concerns.

• Fostering effective communication among family members to minimize conflict and enhance collaborative care planning.
\n
• Providing educational resources to clients and families on community services, support groups, and wellness programs to promote social engagement.

• Maintaining accurate documentation of client interactions, assessment results, and ongoing care plans in compliance with health regulations.

• Staying informed about policy changes and best practices in social work, ensuring adherence to ethical standards and quality care practices.

Patient Population / Client Focus\n

The Senior LMSW will primarily serve the senior population within the community, providing critical support during vulnerable transitions such as health crises, bereavement, and social isolation. By addressing the unique challenges faced by older adults, including mental health concerns and issues related to aging, you will help ensure that they receive dignified, comprehensive care that honors their preferences and values.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• Current, valid licensure as a Licensed Master Social Worker (LMSW) in the state of Texas.

• A minimum of two years’ experience as a social worker, particularly in geriatric or community health settings.

• A deep understanding of social work principles, aging-related concerns, and community resources available for older adults.

• Exceptional skills in assessment, crisis intervention, and counseling in individual and family environments.
\n
• Strong communication and interpersonal abilities, vital for effective collaboration with diverse teams.

• Proficiency in electronic health record (EHR) systems and relevant computer applications for documentation and reporting.

Preferred Experience\n

Experience in community health settings or previous work directly with aging populations is highly valued. Familiarity with telehealth services and electronic documentation practices will enhance your effectiveness in this role. Additional certifications in counseling or geriatric care are considered an asset.

Compensation and Benefits\n
