Title: Physician Assistant Company: Oak Street Health Location: 4600 Capital Blvd, Raleigh, NC 27604 Salary: $126,500 - $148,500 (range depends on experience and location) Role Description The Physician Assistant at Oak Street Health provides effective and equitable value‑based primary care to adults on Medicare. PA’s operate independently and collaborate with physicians and a multidisciplinary care team to deliver primary care, care coordination, and population health services. Core Responsibilities Provide primary care, conducting office visits for routine and acute issues Administer annual wellness visits and health risk assessments with a holistic health focus and accurate, specific documentation Coordinate care with other providers, specialists, testing facilities, and agencies Lead population health initiatives, such as ensuring eligible women receive evidence‑based breast cancer screening every two years Assist the care team with phone triage and outreach Educate patients on their health conditions, care plans, and treatments Participate in Oak Street Health promotional activities Conduct home visits as needed Perform other duties assigned by the Center Medical Director and operational leadership Required Qualifications 3+ years of primary care experience (preferred) Active, non‑probationary state Physician Assistant license Active DEA license U.S. work authorization Demonstrated ability to embody Oak Street Health values of teamwork, innovation, accountability, safety, and quality Preferred Qualifications Experience in internal medicine, geriatrics, or family medicine Understanding of HCC (Hierarchical Condition Categories) documentation, ICD‑10 coding, and Health Risk Assessments (HRAs) Passion for teamwork and cross‑functional collaboration Desire to contribute to an empirically‑guided population health model Bilingual proficiency in applicable languages Pay Range Typical pay range: $87,035 - $187,460. Actual base salary will depend on experience, education, geography, and other factors.
