Admissions Coordinator
Job Summary
Willow Terrace Nursing and Rehabilitation Center, a premier provider of long-term care and rehabilitation services, is seeking a dedicated and compassionate Admissions Coordinator to join our team. As part of our commitment to providing exceptional care, we are looking for a detail-oriented and organized individual to assist with the admissions process and ensure a smooth transition for our new residents.
Admissions Coordinator Responsibilities
Coordinate the admission process for new residents, including gathering and reviewing necessary documentation, conducting tours, and introducing residents to our care team
Work closely with families and healthcare professionals to gather medical and social history information to create a comprehensive patient profile
Collaborate with our health care teams to ensure accurate and timely communication of resident information and needs
Maintain accurate and up-to-date records of resident admissions, transfers, and discharges
Develop and maintain relationships with healthcare professionals, social workers, and other referral sources to ensure a steady flow of referrals
Provide exceptional customer service to residents, families, and healthcare professionals
Admissions Coordinator Requirements
Prior of experience in healthcare administration or a related field
Strong communication and interpersonal skills
Ability to work in a fast-paced environment and manage multiple priorities
Proficiency in electronic medical records software
