Hospice RN Case Manager Per Diem

Ascend Hospice is seeking an RN Case Manager on a per diem basis to support on-call, case management, and field visits. This role involves initial patient assessments, ongoing re-evaluations, and collaboration with the health care team to ensure continuous, coordinated care. The ideal candidate holds a State RN license with at least two years of nursing experience, hospice experience preferred, and must have a valid driver\'s license with reliable transportation. #J-18808-Ljbffr
