As an RN Case Manager, you will be the trusted clinical lead for your patients and their families. You'll manage symptoms, coordinate care, and give families the support and guidance they need during one of life's most meaningful transitions. Responsibilities: Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs Develop, implement, and update individualized plan of care in partnership with patients and families Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care Qualifications: Active Registered Nurse (RN) license in the state you work in Proficiency with electronic medical records (EMR) Strong organizational, communication, and critical-thinking skills Ability to work independently while maintaining high standards of care A compassionate, patient- and family-centered approach Valid driver's license and reliable vehicle with current insurance Benefits: Competitive salary Fleet car program with fuel and insurance covered, for business and personal use *Based on eligibility Medical, dental, and vision insurance Life insurance, Long term/Short term disability insurance Generous PTO package 401(k) with company match $75,000 - $90,000 a year #J-18808-Ljbffr
