CSC'S Case Manager - Behavioral Health ’s Purpose:
Case management services are provided under the direction of, or by referral from, a clinical provider and include client-centered services that implement the clinical treatment plan, including linking clients with health care, psychosocial, and other services. The coordination and follow-up of medical treatments are key components of healthcare case manager.
As a Behavioral Health Case Manager at CSC , You will :
- Work collaboratively with CSC providers to assist clients in meeting their behavioral health goals
- Provide client services at the clinic, field, or telehealth
- Conduct client screenings and assessments to determine service eligibility and appropriateness of case management.
- Verify and support enrollment in medical care, particularly for uninsured and underserved clients.
- Perform client intake, needs assessment, and required documentation
- Assess social, physical, and mental functioning and needs of clients through interviews, home visits, record reviews, or consultation with appropriate agencies and individuals.
- Develop comprehensive care plans in collaboration with clients, addressing short and long-term health goals.
- Maintain regular contact with clients to monitor and facilitate their progress towards their treatment goals
- Refer clients to appropriate services within the care system to promote positive health outcomes and self-sufficiency, ensuring follow-through.
- Provide advocacy and support to clients to assist in their navigation of various health and social systems
- Coordinate care and ensure client is able to access needed behavioral health services within the agency or through various levels of care available in the community
- Assist clients to address barriers related to engagement with behavioral health services such as providing appointment reminders and arranging for transportation
- Collaborate with caregivers, physicians and nursing services, mental health providers, law enforcement, fire departments, and representatives of other service agencies to develop service plans, evaluation, and monitoring of cases.
- Conduct ongoing follow-up, motivational interviewing, and goal-setting with clients and families.
- Deliver education and information tailored to effective client learning.
- Maintain client files according to agency policy and complete documentation promptly.
- Document as needed for the purposes of data collection related to funding and grant needs
- Provide related case management and care coordination services specific to funding and grant requirements
- Participate actively in staff meetings to contribute to clinic development and staff training.
- Travel to programs, events, or sites as needed.
- Perform other job-related duties as assigned based on the needs of BHD/CSC.
Minimum Qualifications:
- Bachelor’s degree required, in a healthcare, social services, human services, gerontology-related field preferred.
- At least one year of experience in a behavioral health-related field or providing case management services preferred
- Bilingual/bicultural English and Chinese (Cantonese and/or Mandarin) or Spanish preferred.
- Knowledge of principles and techniques of social, cultural, and intergeneration dynamics of Chinese, Asian, Latinx, immigrant and refugee communities.
- Comfortable and knowledgeable about the healthcare and/or behavioral health industry.
- Ability to display sensitivity to all cultures.
- Self-motivated, self-driven, and able to work independently within the scope of assigned tasks with minimum supervision.
- Resourceful and flexible in response to changing environments and needs.
- Customer service skills with excellent communication and interpersonal skills, including writing, speaking, verbal, in-person, and via telephone, email, and web posting correspondence.
- Proficient in word processing and data entry using Macintosh, Microsoft Word, Excel, and Outlook.
- Requires standby, flexibility, and/or call-back availability.
- Field work may be required.
- Reliable Transportation.
- Ability to work in a fast-paced, high-intensity environment.
- Conflict management, critical thinking, and problem-solving skills.
- Successfully meet the post-offer/pre-hire physical and receive an “ Eligible for Employment ” status for Level II Background Check required by the State of California.
