Key Responsibilities

• Conduct thorough assessments of residents to identify acute and chronic health issues, ensuring comprehensive understanding of their medical history and current conditions.
• Develop and implement personalized evidence-based care plans that are collaboratively created with fellow healthcare professionals to address residents’ specific health goals.
• Order, interpret, and follow up on diagnostic tests, including lab work and imaging, to inform treatment plans and ensure timely interventions.
• Prescribe medications and therapies with a careful consideration of each resident's needs, documenting all decisions in electronic health records to maintain compliance and continuity of care.
• Educate residents and their families on managing chronic conditions, understanding medications, and adapting to lifestyle changes post-discharge.
• Engage in interdisciplinary team meetings and rounds, contributing insights on patient care and collaborating to optimize outcomes.
• Record detailed notes in electronic medical records, ensuring that updates on residents’ health status are accessible and clear for all care team members.
• Participate in quality improvement initiatives and stay informed of best practices, leveraging the latest healthcare advancements to enhance service delivery.

Patient Population / Client Focus

In this role, you will directly care for an elderly population, often dealing with complex health challenges that include dementia, chronic diseases, and mobility issues. Understanding the unique needs of this demographic is crucial as you’ll be addressing both physical and emotional aspects of wellness. An empathetic approach is essential, as many residents and their families may require support in navigating difficult medical decisions.

Qualifications

• Current certification as a Nurse Practitioner and an active nursing license in the state of Georgia.
• A Bachelor’s Degree in Nursing is required, while a Master’s degree is preferred for advanced practice.
• A minimum of 2 years of clinical experience in a direct patient care role, preferably within an assisted living or geriatric healthcare environment.
• Proficient in electronic medical record (EMR) management and utilization; familiar with common healthcare software systems.
• Exceptional communication and interpersonal skills to effectively engage with residents, families, and team members.
• Strong analytical and problem-solving skills, enabling you to make informed decisions regarding patient care.

Preferred Experience

• Experience working in assisted living facilities or similar environments, specifically serving the elderly.
• Knowledge of geriatric health issues, including cognitive impairment, falls prevention, and general wellness strategies.
• Demonstrated ability to work collaboratively in a team-based care setting.

Compensation and Benefits

The position offers a competitive salary ranging from USD 120,000 to 220,000 annually, commensurate with your experience and qualifications.
