Full-Time Community Health Nurse Practitioner - Assisted Living Facility

Position Overview

The US Veterans Health Care Clinic is seeking a dedicated and compassionate Community Health Nurse Practitioner for a full-time position located in Brookhaven, New York. Key Responsibilities

• Analyze patient health data and conduct comprehensive assessments to develop individualized care plans that enhance overall wellness for residents.
• Implement community outreach programs and health education initiatives to empower veterans and their families with the knowledge and resources to manage their health conditions.
• Maintain detailed, accurate, and confidential medical records in alignment with regulatory standards to ensure high-quality patient care.
• Engage in community health events, offering screenings, preventive services, and wellness education to increase awareness of available health resources.
• Continuously engage in professional development to stay abreast of best practices, evidence-based guidelines, and current health policies relevant to the patient population.

Patient Population / Client Focus

The primary focus of this role is to serve veterans and individuals residing in the assisted living facility. You will address both chronic and acute health conditions, providing culturally sensitive care tailored to the needs of a diverse population. Your role will involve advocating for patient rights and ensuring access to essential healthcare services, thus improving health outcomes in this underserved community.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (ANP) from a recognized credentialing body such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Active, unrestricted Nurse Practitioner (APRN) license in New York State.
• Current DEA and NPI numbers to prescribe and manage medication.
• Demonstrated strong clinical assessment and diagnostic skills relevant to community health.

Preferred Experience

• A minimum of 2 years of clinical experience in community health, public health, or related fields, with strong advocacy and engagement skills.
• Proven ability to communicate effectively with diverse populations, demonstrating cultural sensitivity and adaptability in clinical practice.
• Experience in leading health education initiatives and community outreach programs is a plus.

Compensation and Benefits

This position offers a competitive salary ranging from USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
