Key Responsibilities

• Conduct thorough assessments of residents, identifying acute and chronic health issues through comprehensive examinations and diagnostic evaluations.
• Design and implement individualized, evidence-based care plans in collaboration with a multidisciplinary team, tailoring approaches based on residents' unique medical histories and preferences.
• Order and interpret necessary diagnostic tests, provide timely diagnoses, and prescribe medications and therapeutic interventions as indicated.
• Educate residents and their families on disease management, medication adherence, healthy lifestyle choices, and post-discharge instructions to empower them in their healthcare journey.
• Engage in daily interdisciplinary rounds to discuss resident care, progress, and treatment adjustments, ensuring cohesive communication among the healthcare team.
• Utilize advanced electronic medical record (EMR) systems to document patient interactions accurately, focusing on maintaining precise health records that enhance care continuity.
• Participate in quality improvement initiatives aimed at elevating care standards, sharing insights gathered from hands-on patient experiences, and integrating new research findings into practice.
• Stay abreast of developments in nursing practice, regulations, and healthcare technology, fostering a culture of learning and innovation.

Patient Population / Client Focus

In this role, you will primarily care for elderly residents with a wide range of health conditions, including mobility limitations, cognitive impairments, chronic illnesses, and acute medical needs. Your holistic approach will encompass not only physical health but also emotional and social well-being, allowing you to make a meaningful impact on their quality of life.

Qualifications

• Current and unrestricted certification as a Nurse Practitioner.
• Valid and active nursing license in the State of Georgia.
• A minimum of a Bachelor’s Degree in Nursing or a closely related field; advanced degrees (Master’s) are highly preferred.
• A minimum of 2 years of clinical experience in a healthcare setting, preferably within assisted living or related environments.

Compensation and Benefits
