Work Environment

This position requires an onsite presence at our assisted living facility located in Atlanta, GA. Our work environment is designed to facilitate effective teamwork and communication among staff, fostering a culture of support and camaraderie. With a flexible weekly schedule of Monday, Wednesday, and Friday from 8:00 AM to 5:00 PM, we aim to accommodate the work-life balance of our team members while ensuring the delivery of high-quality healthcare services.

Key Responsibilities

• Provide comprehensive primary care services tailored to the unique needs of our patients in an assisted living setting.
• Perform health risk assessments to identify and address potential health issues, utilizing evidence-based practices.
• Deliver in-depth wellness education to promote preventive measures, empowering patients to make informed health choices.
• Assist patients in managing chronic conditions effectively through personalized care plans and follow-up consultations.
• Lead initiatives to ensure that quality and safety standards are met within our healthcare facility.
• Build and maintain collaborative relationships with interdisciplinary team members and support staff to enhance care coordination.

Patient Population / Client Focus

The Family Nurse Practitioner will primarily serve a population of elderly patients residing in the assisted living facility. These individuals often have complex health needs resulting from age and chronic conditions. Therefore, the FNP will play a critical role in promoting health, preventing disease, and enhancing the quality of life for our residents through comprehensive care management.

Qualifications

• Master’s Degree in Nursing is required.
• Current and valid Family Nurse Practitioner License to practice in Georgia.
• Strong understanding of company standards of operations and compliance with healthcare regulations.
• Excellent interpersonal skills with an ability to maintain positive working relationships with patients, families, and medical staff.
• Proficient in providing care through telehealth platforms, as this role includes virtual consultations.
• Demonstrated ability to perform efficiently in a fast-paced and autonomous environment.

Preferred Experience

In addition to the qualifications, applicants with prior experience as a Telehealth Clinician will be given preference. Familiarity with electronic medical record (EMR) systems and previous work in a similar healthcare setting, particularly focused on geriatric care, will be advantageous.

Compensation and Benefits

This position offers a competitive annual salary range of USD 120,000 to 130,000. Furthermore, our benefits package includes:
