Work Environment

As a Family Nurse Practitioner at Lifeline Family Care Center, you will operate in an onsite health and wellness center that complements house call services. Your work schedule will typically be Monday, Wednesday, and Friday from 8:00 AM to 5:00 PM, providing a consistently structured environment with no on-call duties or weekend shifts. Key Responsibilities

• Conduct thorough health assessments and evaluations, facilitating comprehensive primary care services tailored to each patient’s needs.
• Perform health risk assessments to identify potential health concerns and develop individualized intervention strategies for effective management.
• Deliver educational resources and guidance to patients on promoting wellness, preventive measures, and adopting sustainable healthy lifestyle choices.
• Assist patients in managing chronic conditions by developing and implementing effective treatment plans alongside ongoing monitoring for optimal care.
• Coordinate care with the medical team to ensure seamless patient experiences and integrated healthcare services within the facility.

Qualifications

Compensation and Benefits
