Work Environment

As a Nurse Practitioner, you will work onsite, directly interacting with patients and families in their homes across Atlanta. Key Responsibilities

• Conduct detailed assessments and form diagnoses of patients' acute and chronic medical conditions in their home settings.
• Develop, implement, and manage evidence-based treatment plans tailored to individual patient needs, collaborating closely with physicians and other healthcare professionals.
• Order and interpret diagnostic tests, and prescribe medications and therapies as appropriate to ensure optimal health outcomes.
• Educate patients and their families regarding disease management, medication adherence, and effective post-discharge strategies.
• Participate in interdisciplinary team meetings, contributing to the integrated care plans that guide our approach to health management.
• Document patient interactions and treatment plans accurately in electronic medical records, ensuring compliance with healthcare regulations and standards.
• Engage actively in quality improvement initiatives, adhering to evidence-based practice guidelines to refine and elevate our service quality.

Qualifications

Compensation and Benefits

The compensation for this role is competitive, with an annual salary range between USD 120,000 and USD 220,000, commensurate with your experience and qualifications.
