This position will take place at our onsite facility dedicated to home care in Atlanta, GA. Key Responsibilities

• Conduct comprehensive initial patient histories and physical examinations to assess health status.
• Develop, document, and communicate individualized care plans tailored to each patient’s health requirements, ensuring thorough screening and preparation.
• Perform daily patient rounds and monitor ongoing conditions, adjusting care plans as necessary.
• Prescribe medications, laboratory tests, and diagnostic procedures, ensuring proper documentation of orders.
• Interpret test results and laboratory findings, discussing outcomes with patients to optimize care.
• Collaborate closely with attending physicians, residents, nursing staff, and interdisciplinary teams to ensure high-quality patient care.
• Write detailed discharge summaries, prescriptions, and referrals, providing patients with post-discharge care instructions to facilitate recovery.
• Participate actively in continuous education and training to enhance skills and stay current with medical advancements.

Patient Population / Client Focus

As a Women’s Health Nurse Practitioner, you will work with a diverse population of patients, providing care that addresses a wide range of health concerns from adolescence through menopause and beyond. Qualifications

• Master’s Degree from an accredited Nurse Practitioner program is required.

• Current Georgia state license to practice as a Nurse Practitioner and Registered Professional Nurse.
