Work Environment

This position is primarily onsite, allowing you to interact directly with patients in their homes. You will work a standard 40-hour week, Monday through Friday, with a focus on delivering in-depth, personalized care in the comfort of the patient’s environment. The collaborative nature of our practice includes working closely with five physicians and a peer group of five Nurse Practitioners, promoting a culture of teamwork and shared expertise. In this role, you will engage with patients from diverse backgrounds, requiring a sensitivity to cultural nuances and a commitment to equitable care.

Key Responsibilities

Your primary responsibilities will include:

• Conducting thorough physical examinations and in-depth health assessments of patients.
• Interpreting laboratory test results and diagnostic imaging, such as X-rays, ensuring accurate medical diagnoses.
• Formulating and implementing comprehensive treatment plans for chronic and acute health conditions.
• Prescribing necessary medications and managing ongoing patient care.
• Maintaining meticulous patient records, schedules, and documentation, enhancing continuity of care.
• Informing patients about their health status as well as education on prescribed medications.
• Promoting healthy lifestyle choices through targeted patient and family education.
• Leading initiatives that advance patient health literacy and self-management strategies.
• Engaging in ongoing research and professional development to stay abreast of healthcare advancements.

Patient Population / Client Focus

As a Nurse Practitioner at Heart Care Center PC, you will serve a diverse patient population, including individuals with chronic illnesses, acute medical conditions, and those requiring preventative care. This role will enable you to impact their lives significantly, ensuring that each patient feels cared for through attentive, thoughtful interactions that emphasize wellness and healthcare education. You will assist patients facing complex health issues, helping to navigate their treatment paths effectively.

Qualifications

The ideal candidate will hold a Master’s degree in Nursing and possess a valid Nurse Practitioner license in New York State (ANP/FNP) with an active DEA number. You should demonstrate proven experience as a Nurse Practitioner in various settings, such as clinics, hospitals, or emergency rooms. Knowledge of best practices in patient care and health regulations is essential, alongside exceptional communication skills and a commitment to ethical healthcare delivery.

Preferred Experience

We highly value candidates who have clinical experience in any of the following specialties: acute care, internal medicine, cardiology, family practice, urgent care, or emergency department settings. Compensation and Benefits

Heart Care Center PC offers a competitive salary range of USD 140,000 to 180,000 per year, commensurate with experience, along with a comprehensive benefits package that includes:
