Work Environment

Key Responsibilities

• Conduct thorough initial patient histories and comprehensive physical examinations to establish a foundation for care.
• Develop, document, and communicate personalized care plans, ensuring all aspects of patient preparation and screening are meticulously followed.
• Perform daily patient rounds to monitor progress, evaluate responses to treatment, and adapt care plans accordingly.
• Write orders for necessary medications, laboratory investigations, and diagnostic tests, ensuring accuracy and clarity in all communications.
• Interpret a range of laboratory and diagnostic test results, using clinical judgment to inform treatment decisions.
• Collaborate with physicians, residents, nursing staff, and other care providers to enhance the quality and consistency of patient care.
• Create detailed discharge summaries and ensure that patients receive and understand their post-discharge care instructions.
• Engage in performance improvement initiatives, research, and educational activities to maintain and enhance the quality of care provided.
• Perform other related duties as necessary, in alignment with the essential functions outlined under the Americans with Disabilities Act.

Patient Population / Client Focus

As a Nurse Practitioner in Internal Medicine within our home care setting, you will primarily serve adult patients managing chronic and complex health conditions. Your role will involve actively engaging with patients who may have limited mobility or require specialized assistance due to their medical needs. Your compassionate approach will be vital in enhancing their health outcomes and quality of life.

Qualifications

• A Master’s Degree in a Nurse Practitioner Program is required, along with national board certification in the specialty area.
• A valid Nurse Practitioner license and a Registered Professional Nurse license in New York State are mandatory.
• Evidence of a collaborative practice agreement with established protocols filed with the New York State Department of Education, or alternative collaborative relationship attestation in accordance with the Nurse Practitioner Modernization Act of 2015.
• Current BLS (Basic Life Support) certification is essential.

Preferred Experience

• Experience in home care or community health settings is highly valued.
• Proficiency in managing patients with chronic illnesses, including but not limited to diabetes, hypertension, and heart disease.
• Strong communication skills and the ability to build rapport with patients and families in a home environment.

Compensation and Benefits

This position offers a competitive salary ranging from USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
