Key Responsibilities

• Conduct thorough assessments by performing and documenting comprehensive medical, surgical, and psychosocial health histories and examinations, identifying patient needs and establishing treatment plans.
• Utilize diagnostic skills to order and interpret necessary laboratory tests and imaging studies, leading to informed clinical decisions.
• Facilitate hospital admissions and discharges, ensuring smooth transitions for patients while coordinating inpatient and outpatient care, including home healthcare services.
• Engage in effective patient education by providing counseling on disease processes, management strategies, and preventive health measures tailored to individual patient circumstances.
• Lead phone triage for acute clinical issues and conduct assessments as per established protocols, ensuring timely interventions.
• Assist in developing and evaluating standards of care, ensuring alignment with institutional guidelines and regulatory requirements.
• Mentor and delegate responsibilities to support staff, fostering a collaborative team approach to patient care.

Patient Population / Client Focus

As a Nurse Practitioner, you will serve a diverse array of patients, addressing both acute and chronic health conditions. Qualifications

• Current New York State Registered Nurse (RN) License.

• Current DEA License and BCLS/ACLS certification.

Compensation and Benefits
