Key Responsibilities

• Perform thorough health assessments and screenings for patients, developing tailored care plans based on individual needs.
• Diagnose and manage a range of acute and chronic conditions, with a strong emphasis on preventive care and health promotion strategies.
• Work collaboratively with community organizations to design and implement effective health education programs that empower patients and enhance overall community health.

Patient Population / Client Focus

In this role, you will serve a diverse patient population encompassing various ages, backgrounds, and health conditions. Our primary focus is to support individuals and families who may face barriers to healthcare access, fostering inclusive practices and promoting health equity through our care approach.

Qualifications

• Master's Degree or Doctoral Degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Active, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possess a current DEA# and NPI#.
• Strong communication and interpersonal skills, with the ability to build relationships with patients and team members alike.
• Experience using electronic health records (EHR) systems effectively.

Preferred Experience

• A minimum of 2 years of clinical practice experience, particularly in primary care or family medicine settings is preferred.
• Experience working in community health or public health environments is advantageous.
• Proven clinical assessment skills and diagnostic proficiency.

Compensation and Benefits

• We offer a competitive salary ranging from $150,000 to $160,000 annually, commensurate with your experience and qualifications.
