Work Environment

The clinic operates on a structured schedule, with working hours typically from Monday to Friday, either 9 am to 5 pm or 8 am to 4 pm, allowing for a healthy work-life balance.

Key Responsibilities

As a Nurse Practitioner at our clinic, you will be responsible for a variety of essential functions:

• Conduct comprehensive physical exams and evaluate patients presenting with acute, episodic, or chronic conditions.
• Diagnose medical illnesses and develop tailored treatment plans, ensuring that each patient receives personalized and effective care.
• Prescribe medications responsibly and manage medication lists according to the latest clinical guidelines.
• Engage in dialogue with collaborating physicians and specialists to facilitate seamless patient referrals and comprehensive care.
• Document patient care accurately using our electronic health record system, ensuring continuity and coordination of care.
• Implement and modify patient management policies, contributing to the overall improvement of clinic operations.
• Educate patients and families on health management, providing resources and guidance to foster independence and wellness.

Patient Population / Client Focus

Our primary patient population consists of elderly individuals residing in our Assisted Living Facility, who often require urgent care for both minor and complex medical conditions. You will be tasked with understanding the unique challenges faced by this demographic, including managing chronic diseases and the need for complex care coordination. Your empathetic approach and expertise will support their health-related needs, ensuring a swift recovery and a higher quality of life.

Qualifications

• A Master’s degree in Nursing with a specialization as a Nurse Practitioner (NP).
• An active state Nurse Practitioner license in New York.
• A minimum of 2 years of experience in a healthcare setting, preferably in urgent care or geriatric care.
• National certification as a Nurse Practitioner with prescriptive authority recognized across the United States.
• Proven knowledge of medical terminology and pharmacology.
• Strong communication skills, capable of leading patient consultations and nurturing team collaboration.
• A commitment to maintaining professionalism, demonstrating sensitivity to the needs of patients and their families.

Preferred Experience

• Experience with electronic health record (EHR) systems, particularly in an urgent care or assisted living context.

Compensation and Benefits
