Work Environment

This position is based onsite at our outpatient office in Brookhaven, NY, where collaboration and teamwork are at the heart of our operations. Your typical work schedule will be from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics until 8:00 p.m. and participation in community outreach events on weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings for diverse patients, ensuring the collection of relevant medical histories and lifestyle factors to inform care plans.
• Diagnose and manage both acute and chronic medical conditions, utilizing evidence-based guidelines to provide preventive care and promote health education.
• Collaborate with local health organizations to create and implement educational programs tailored to the community's specific health challenges.
• Advocate for patient rights and access to healthcare, working diligently to eliminate disparities in care and improve health literacy among community members.
• Coordinate referrals and follow-up treatments effectively, ensuring seamless continuity of care across various health services.

Qualifications

• Must possess current certification as a Nurse Practitioner and hold an active nursing license in the state of New York.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 per year, commensurate with your experience and qualifications.
