Atlantis Athletes Physical Therapy Care is seeking a qualified and compassionate Nurse Practitioner (NP) to join our dedicated team in Brookhaven, New York. Work Environment

This position is onsite, with a collaborative work environment designed for the best outcomes for our patients. You will be working alongside a multidisciplinary team that includes physicians, physician assistants, registered nurses, and dedicated medical scribes. This collaborative care team operates within outpatient settings, focusing solely on delivering primary care, care coordination, and population health activities. Your insights and clinical expertise will directly contribute to achieving the organization's clinical goals and enhancing patient experiences.

Key Responsibilities

• Deliver exceptional primary care services, addressing both routine and acute medical issues through thorough assessments and evidence-based practices.
• Conduct comprehensive annual wellness visits and health risk assessments, requiring a meticulous focus on documentation and a holistic view of patient health.
• Facilitate care coordination with external providers, specialists, and testing facilities to ensure continuity of care.
• Lead population health initiatives, ensuring timely screenings, including evidence-based breast cancer screenings for eligible patients every two years.
• Assist the care team with phone triage and patient outreach, promptly addressing patient concerns and health queries.
• Engage with patients in educating them about their health conditions, treatment options, and care plans to promote informed decision-making.
• Participate in marketing and promotional activities to enhance community awareness and accessibility to our services.
• Conduct home visits as necessary to provide care to patients who may have difficulty access to an outpatient setting.
• Report to the Medical Director and collaborate closely with operational leadership to align clinical practices with organizational goals.

Patient Population / Client Focus

Your primary focus will be on providing compassionate care to older adults on Medicare. By understanding their unique health needs and life circumstances, you will help ensure they receive the necessary resources and support to maintain their quality of life. Your role in establishing meaningful connections with patients will be essential in empowering them to take control of their health.

Qualifications

• A Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized bodies such as AANP or ANCC.
• An active and non-probationary state Nurse Practitioner license with a current DEA license.
• Demonstrated experience in primary care, specifically internal medicine, geriatrics, or family medicine.
• Proficient understanding of HCC documentation, ICD-10 coding standards, and Health Risk Assessments.
• A strong passion for teamwork, with an emphasis on collaborative practices across various healthcare disciplines.

Preferred Experience

• Experience working in a concierge medicine setting or similar environments focused on personalized health care delivery.
• Previous exposure to healthcare management systems and electronic medical records that enhance patient care coordination.
• A history of utilizing innovative approaches to patient education and care delivery.

Compensation and Benefits

The compensation range for this full-time Nurse Practitioner position is between USD 120,000 and 160,000 annually, depending on relevant experience and qualifications.
