Key Responsibilities\n

• Develop and maintain individualized treatment plans by conducting extensive psychosocial assessments that address the unique concerns, strengths, and needs of patients and their families.

• Regularly assess and adjust treatment strategies based on ongoing evaluation of patients' emotional and psychosocial health, respecting their cultural and spiritual values.

• Lead educational and therapeutic group sessions, documenting patient engagement and progress systematically to assess treatment efficacy.

• Participate in treatment planning meetings to collaborate with multidisciplinary teams, providing insights into patient progress and challenges while updating treatment plans accordingly.

• Act as a liaison for community resources, enhancing patient access to necessary services and support post-discharge.

You will be working primarily with patients transitioning into hospice care, along with their families, helping them navigate the emotional and practical aspects of end-of-life care. Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Active and unrestricted licensure as a Clinical Social Worker (LCSW) in California is mandatory.

Compensation and Benefits\n
