RN Case Manager

As a Registered Nurse (RN) Case Manager at HCA Healthcare Trident Hospital , you will have staffing support, technology and resources to deliver safe, high-quality care. Job Summary And Qualifications As a Registered Nurse (RN) Case Manager, you will support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care. Responsibilities Coordinate the plan of care for the patient stay, manage the length of stay, ensure appropriate resource management, and develop a safe, appropriate discharge plan. Facilitate progression and transition of care and identify patients at risk for adverse outcomes during transitions between levels of care/settings. Perform a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, and assess available resources; reassess the patient’s clinical condition as indicated. Ensure post-acute medical needs and level of care are appropriate in partnership with Social Services. Evaluate progression of care using evidence-based tools and approved criteria (InterQual); escalate progression and transition of care issues as needed. Make appropriate referrals to third-party payer, disease and case management programs for recurring patients and patients with chronic disease states. Act as a liaison through effective and professional communications between physicians, patient/family, hospital staff, and outside agencies; advocate for patient rights, needs and values. Qualifications Graduate from an accredited school of Nursing: Associate’s Degree in Nursing or higher required; BSN highly preferred. Licensed as a Registered Nurse in the practicing state and/or hold an active Compact/Multi-State RN license.

1. 3 years of clinical, hospital nursing experience required.
2. years of experience in Case Management highly preferred.

Certification in Case Management or Utilization Review preferred. InterQual experience preferred.
