Certified Patient Care Coordinator

Managing the cardiac remote monitoring program, the full-time Certified Patient Care Coordinator will review daily alerts and transmissions from cardiac devices, document findings, and communicate with patients and clinical staff in a remote setting. Key responsibilities: Review and analyze remote monitoring software for patient transmission data and alerts Coordinate device monitoring programs and ensure appropriate communication with clinical staff and patients Educate patients on remote monitoring and maintain knowledge of new developments in device technology Required qualifications: International Board of Heart Rhythm Examiners (IBHRE) certification preferred; Certified Rhythm Analysis Technician (CRAT) certification required prior to employment Strong knowledge of cardiac devices and rhythm interpretation Experience with remote device transmissions from major manufacturers such as Abbott and Medtronic Demonstrated ability to work independently and exhibit strong leadership skills Understanding of HIPAA regulations and commitment to patient confidentiality
