Care Management Coordinator

Care Management Coordinator – Case Management Job Location: Covenant Medical Center, 19th St, Lubbock, TX. This is an onsite position that requires commuting. Job Schedule: Full time, Day Shift, 40 hours per week. Job Responsibilities: The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, the coordinator shall coordinate communication among patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care and payment for clinical services. The coordinator will maintain accurate records, process documentation, and support the delivery of quality patient care. Required Qualifications Minimum 2 years of administrative experience, preferably in a healthcare setting or with demonstrated knowledge of healthcare terminology. Preferred Qualifications Bachelor's Degree or higher in Business, Healthcare, or a related field.
