REGISTERED NURSEThe West Palm Beach, FL market is in need of a Home Health skilled RN-with Oasis experience we can do this several ways:JOB DESCRIPTIONREPORTS TO: Director of NursingJOB SUMMARY:Professional member of home health team who provides skilled nursing visits in patient's home under the direction of plan of treatment established with physician and input from patient/care giver.DUTIES AND RESPONSIBILITIES:Provides initial and on-going assessment of client needs using the OASIS data elements incorporated into the comprehensive assessmentWith input from patient/care giver and in conjunction with physician, formulates and implements plan of careEvaluate effectiveness of care plan and make necessary adjustmentsProvides for the emotional and physical comfort and safety of client taking into consideration their rights and cultural backgroundReceives and transcribes physician ordersNotifies physician and agency supervisor of unusual reactions and/or changes in client's conditionDocuments all appropriate observations and treatments in keeping with agency policies and proceduresParticipates in case conferences, team meetings, staff meetings, and performance improvement activities as assignedProvide any skilled nursing service for which appropriately trained which is prescribed under plan of care. Prepares clinical progress notes as per agency policies and proceduresProvide 60 day summary of skilled services and client outcomes to physician and agency supervisor in keeping with agency policies and procedures.Adheres to all agency policies and procedures including but not limited to the HIPAA privacy rule.Maintains strict confidentiality of all patient, employee and agency informationOther duties as assigned by supervisor and for which he/she is qualifiedConducts self in professional manner at all times and in all situationsParticipation in all agency in-service programsQUALIFICATIONS:Must be a graduate of an approved school of nursingCurrently licensed in the state of the HHASign on of $10,000 or relocation fees and sign on of $5000(FT includes phone and internet allowance of $50 each per month, tablet for documentation, mileage of .50/mile) Start date asap
