Position Overview:

The Home Health Physical Therapist (PT) is responsible for evaluating patients, developing individualized treatment plans, and providing skilled physical therapy services directly in patients' homes.

This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while collaborating with an interdisciplinary healthcare team.

Key Responsibilities:

Patient Evaluations:

• Perform comprehensive patient assessments and evaluations.
• Identify physical limitations and functional deficits.
• Develop individualized treatment goals and care plans.

Treatment & Patient Care:

• Implement personalized therapy programs designed to improve mobility and function.
• Provide skilled therapeutic interventions in the home setting.
• Monitor patient progress and modify treatment plans as appropriate.
• Assist with discharge planning and continuity of care.

Documentation:

• Complete accurate and timely clinical documentation.
• Utilize Homecare HomeBase (HCHB) and OASIS documentation systems.
• Maintain compliance with all clinical and regulatory standards.

Collaboration & Supervision:

• Communicate effectively with interdisciplinary care teams.
• Coordinate patient care plans across disciplines.
• Provide direction and oversight to Physical Therapist Assistants (PTAs)

Requirements:

Experience:

• Minimum 2 years of Physical Therapist experience REQUIRED.
• Minimum 1 year of Home Health PT experience REQUIRED.
• Homecare HomeBase (HCHB) experience REQUIRED.
• OASIS documentation experience REQUIRED.
• Prior travel therapy experience strongly preferred.

Education:

• Bachelor's degree in Physical Therapy REQUIRED.
• Master's or Doctorate preferred.

Licensure & Certifications:

• Active Hawaii Physical Therapist License REQUIRED.
• Active BLS Certification (AHA) REQUIRED.
• CPR Certification REQUIRED.
